• Added a :kura_cache_opt_out account-level feature flag helper.

• Denormalized project_id onto ClickHouse cas_outputs and populated it for new CAS output writes, seeds, and test fixtures.

This keeps the max rolling window at 1,000, but removes the expensive full-project ClickHouse scan from the steady-state path for flaky-test alerts.
