Redesigned Identities page as a Noora providers table

The account Identities page now renders as a full-width Noora card with a providers table (Provider / Account / Status columns), matching the products settings page. Each provider is shown with a Connected badge and linked account id when linked, a Connect button pointing at /auth/ when the instance has the provider configured but the user has not linked it, or a dimmed Not configured badge for providers the instance has not enabled.
