Durable action evidence persisted in once.sqlite

Once now persists durable action evidence in a SeaORM-backed once/once.sqlite workspace store. Run, exec, and graph build / run / test outcomes are recorded as evidence records and surfaced to users through a new once query evidence command, plus an MCP evidence query tool. This gives users a queryable memory of what their project graphs have actually done.
