Show clear errors when Slack install fails

When Slack OAuth fails, you now see a clear on-screen error instead of a silent failure. Common cases, such as a denied/cancelled install or a missing authorization code, are handled with explicit messages. This includes the new flash-toast infrastructure so notifications stay visible across pages.
