Slack repeated installs and workspace UI improvements

Slack workspace installs now work reliably when multiple users or the same user try to install again. The authorization flow replaces the session-based state check with a signed, user-bound token and friendlier error messages when a link expires or belongs to another session. The account Slack page also shows connected workspaces, empty-state guidance, and disconnect actions in a cleaner layout.
