Persist durable action evidence in once.sqlite

Action evidence from once exec, the legacy once run path, and the graph build / run / test flow is now persisted in a SeaORM-backed .once/once.sqlite workspace store with an evidence_records table. Inspect it through the new once query evidence command or via the MCP evidence query tool.
