Configure instance visibility with HIVE_VISIBILITY

Hive server 0.1.0 can be deployed as a public or private instance. Use the HIVE_VISIBILITY environment variable to set public access (anyone can use the dashboard without signing in) or private access (dashboard routes require authentication).
