Polished Slack workspace management UI

The Slack account page has been redesigned to make workspace connections easier to scan and manage. Each workspace now shows its name, connected or disconnected status, installer email, and install date in a dedicated row, with a more prominent disconnect action. Empty states are clearer when Slack is not configured or no workspaces are connected, and the layout adapts better on smaller screens.
