Repeated Slack workspace installs are now supported

Slack workspace installations can now be repeated reliably. Previously, reinstalling a workspace could fail because the OAuth state was tied to the browser session. The install flow now uses a signed, user-bound token to verify the callback, so you can reinstall or reconnect a Slack workspace without running into stale-session errors.
