Slack install failures are now visible

Installing the Slack app no longer fails silently. The OAuth callback now validates Slack’s response and surfaces clear error messages when the install link expires, Slack rejects or cancels the request, or the authorization code is missing, so you can retry or fix the setup without guessing what went wrong.
