Support for connecting multiple Slack workspaces

Users can now connect multiple Slack workspaces to Hive from the account Slack page instead of being limited to a single install. Each workspace appears as a separate connected installation with its name, install date, and the installer shown, and active installs can be disconnected directly from the list. This makes it easier for teams to manage Slack integrations across different workspaces.
